I'd recommend showing 5 random popular/good rated mods instead of the very top 5. Seeing the same mods over and over again on the front page looks boring and shadows many other good mods.

Yeah that went really well the last time.
Desura was a small-time game distribution platform (not really a mod app) = you can't contend against Steam/Valve.
Maybe a open source based approach that is focused on one game per engine with examples and instructions would be a good way to start.
But it would still need to have a clever design and good logic and take a lot of work to get some where at the beginning.
A mod manager for this site could really gain ground with the likes of Nexus Vortex mod manager. Seeing how mod authors at the moment are currently jumping ship from that site due to disagreements with the site owners and people feeling blindsided by the sudden change in policy, such a move might help corral them back into one unified spot rather than having to search through personal discords and other outlets to find useful mods.
I first ticked "payment system" then chickened out and chose "mod manager" :P
The last thing I want from you is some app.
A mod manager would be great, though I feel like if you could provide an API the community could run with that and even integrate it into other projects or games directly.
An ability to filter out specific games when searching
With the rise of Workshop, Creation Club and such like DMR'd proprietary platforms, which create a two tier gaming system, I would sugest removing any mod which is only tied to one of those platforms. I have a few times come here only to find a mod is nothing more than an advert for the closed Workshop platform.
I also fully agree with redismonDE, the last thing needed is another garbage client software on top of the hundreds already in existance. Promoting the use of guides and lists (such as on nexus for Elder scrolls) would be more beneficial. A prime example is the nexus mod manager, I don't know of anyone who uses it or wants to, there are better options and its just a waste.

Donation (payment) system could be the differential thing that could you make stand apart and shine.
Not again, I don't need another launcher with tons of ads and other unused garbages with forced auto-update every week. More mods, better comment threading and bookmarks are better. Hell, if truely possible flash revival would be awesome. I miss those games.
i would say several stuff in the poll (if possible) also, the mesage system could be better. It should work more like a chat thing with immediate messages than the current system

Comment notifications would by far be the biggest improvement for me as a mod author. I have to revisit all the important pages manually. I literally have a routine for it by now. And I still miss some comments.
And the payment systems idea is a road to hell.
Would be great having the possibility of posting more videos or images when we post an addon for an existing game !!!! I just put mine and I can't put any images or videos like a new MOD, I don't like it...

The whole image handling system needs an overhaul (to slow/better handling).
Also the video player(upload/play) seems to be to slow.
Moddb doesn't need a mod manager. Let me manage my own mods. I'm gonna echo the comments of X1Destroy and Cvoxalury.
One of things I love about moddb is that, upon returning after a decade away, I find exactly the same site, and a strong modding community to this day. That shows the dedication of its fanbase. At this point in my life I'm old and arthritic and any major changes will probably scare me away (although not really because where the hell else am I gonna go?)
