This is the CD-rom that came with the August 1996 issue of PC Zone from Dennis Publishing, Included in this Disk is demos of Witchaven 2, Quake (First Episode), PowerSlave, and other games, Also as a bonus is a Duke Nukem Screensaver and 101 Duke Nukem 3D Maps (split between Dukematch and Singleplayer), This CD was repeatedly flagged as a trojan horse but it's verified as safe, It comes from archive.org and I don't own or take credit for it either.
