My house rules
1. There is only one weapon shop with an armoury per map.
2. No bus service.
3. No weapon crates.
Alternatively, after purchasing a crate use vehicles to move it from the weapon shop to a safehouse (roleplay, imagine). If purchased at separate occasions, these are separate trips. Also move them between safehouses.
4. No syringe use, only emergency self-surgery at critical health and rest anytime (always maximum length).
5. Buddy does not magically appear to save me. (Maybe one time but don't reactivate.)

Mainly and specifically:
a. no location marker on the map
b. delayed respawning
c. no automatic reloading of weapons if empty
Changes I would welcome
α. enemies retake safehouses (respawn)
β. randomized respawn time (20-50 minutes)
γ. no diamond suitcase sensor, no proximity sound
δ. ability to buy the manuals without having to buy the weapons first
Not feasible:
ε. assign any item to any slot
ζ. small groups of unarmed civilians moving between places on foot and in vehicles (G.O.R.E. Moddb.com replaced patrols with roaming civilians)
η. storage in cars
