Which icon?
I haven't added joints, but it's possible since I did use a hud mod for the guitar that also adds cigarettes, and I am considering the idea since I'm not really big on cigarettes.
The icons for the food items from left to right are:
cheese
dried fruit
tea
instant coffee package
bottled water
energy bar

Do we need to drink water now or water just removes hunger like bread?

You probably won't need to "drink" water, even though you should, but it's going to have some beneficial properties that will be better than most foods. It's very likely that it will restore a small amount of health, and stamina in addition to removing hunger like food.
I think a dehydration mechanic would be great, but it would require a script to implement. Something like death if you don't drink water for X number of hours, with a message saying you died of thirst.
Might want to saturate the Obrez texture - it looks a bit weird, compared with the rest.
