His physical health might have been restored but he would never be the same mentally.

Not having seen that TOS episode, when I read the bit about him losing his "two left limbs" I assumed they were both arms for some reason. 😂
A four-armed hero Starfleet captain would've been pretty dang cool!

lol - Captain Goro!

There are varying accounts of how Garth of Izar ended up going dangerously insane, and most are quite vague. Some say he caught some kind of disease, some say he was injured in an accident, some say he was wounded in some nameless battle. We decided to go our own route and say he met his downfall at the Battle of Axanar.

Description

USS Xenophon approaches the peaceful, remote world of Antos IV, carrying aboard the scarred and deranged Kelvar Garth of Izar.

Garth's historic and tide-turning victory at the Battle of Axanar had come at terrible cost. In the Xenophon's final attack run on the Klingon flagship, an explosion on the bridge had removed Garth's two left limbs and left him with a critical brain injury that even 23rd Century medicine had struggled to cure. Comatose for months after the battle, when Garth regained consciousness he was a changed man - still brilliant and charismatic, but prone to extreme paranoia, elaborate delusions of grandeur, and a dangerous, even sadistic predisposition towards manipulation and violence. His limbs could be replaced with advanced prosthetics, but even under the very best care at Starfleet's disposal, it was clear that Garth of Izar would never command a starship again.

Dismayed, the crew of the Xenophon begged Starfleet Command to allow them to take Garth to Antos IV. Though far from the Federation, the Antosians were famed for their ability to heal even the deepest injuries and the most virulent diseases. If hope for Garth remained, it would be on Antos.

Starfleet agreed. Garth was not only an accomplished captain, but his successes against the Klingons, especially at Axanar, had made 'Garth of Izar' a household name across the Federation. The Xenophon was placed on detached duty, and allowed to embark on the long journey to Antos IV.

The Antosians spent many days alone with Garth, granting him their abilities of cellular metamorphosis and teaching him to control it. Garth's scarred body was rejuvenated, and his missing limbs grew back healthy and strong, as though his injuries had never occured. Garth was restored to virtually perfect physical health.
