They are so named the Devout for their unwavering loyalty to Sauron, proven by their scars and fearlessness.

Are the similarities to the Mouth of Sauron a coincidence, or is there some connection?
No, it was inspired by the Mouth. After all, he was one of the few surviving Black Numenoreans under the rule of Sauron during the Third Age, so it was appropriate to use him as a template.
Can't you make ingame shots from these guys? Not that I dislike this picture, but ingame we get a better look of their appearance
