Seen here with the interface chosen to interact with the "War Doctor".

Everyone else feared it because it could destroy anything and anyone. The Time Lords feared it because it became sentient and would possibly judge (and thus implied punish) anyone or group who used it.

The doctor, driven to despair by the time war, chose to do so.
Through his 9th, 10th and 11th regenerations we have heard about the end of Gallifrey and the daleks. The Doctors punishment was survival.

We are not given any specific details of operation. Just the Doctors assertion that it have a "Big Red Button." We can conjecture that it is a time enabled weapon because it eliminates the chosen target in a single moment and can even time lock it.

Which makes little sense of all the Doctors talk of "They all burned."
Who knows, perhaps the act of acquiring the target is similar to burning something. Perhaps its surrounded by something like st. elmos fire, and the Doctor witnessed this. If all went in a single moment, the target(s) certainly would feel, nor be aware of anything.
