The shapes of puddles are randomized and they grow in size according to how much blood the bleeding character loses. This is playing at 5x the normal speed.

Last game i remember, that had good looking blood pools, was Max Payne 3.
I can get away with it because the levels are small thanks to being stacked only one room deep. It eats a lot of texture memory. But with some additional tricks I don't see why it wouldn't work in full 3d too.
