This is the model of X1, from the games "Star Wars: Elite Squadron" on the PSP and Nintendo DS, and the unreleased game "Star Wars: Battlefront III" from Free Radical.
This is the genetic clone of the Jedi "Falon Grey", he is also the twin brother of Clone Trooper (and later Jedi Master) "X2".
He and his brother X2 were the ones responsible for training almost every clone trooper in the entire clone army.
He became a Sith Lord after being a clone trooper as well as a stormtrooper and is the one who took over the Imperial Remnant directly after Palpatine died, captured Luke Skywalker, and eventually was killed by his brother X2 on Mustafar.

The model itself is taken from the cancelled game "Battlefront III", making it the most aesthetically pleasing model of the character.
