Localized Country Names is a mod which seeks to localize every country's name in Victoria 2. Using Wikipedia, google translate, some online dictionaries, and other peoples advice, I have compiled all of the names into one mod.
This mod is compatible with:
- Vanilla
- HPM
- HFM
- HBM
- GFM
- TGC

You have to download the file first and then unzip it. Once you have completed that, go to your Victoria 2 folder and then open the file called "localisation". Place the file inside that folder. If you want to use this with other mods, just open the folder called "mod" and then select the mod with which you want to combine with this mod, and placed this mod inside that mod's "localisation" folder.

I used HBM in very first file I posted, followed by TGC in the next 3, and GFM for the newest file I posted.

So I've looked through Polish countries since I'm Polish and here're my comments:
Galicia-Lodomeria:
- Republika Galicja-Lodomeria -> Should be "Rzeczpospolita Galicji-Lodomerii"
- Polska Rzeczpospolita Galicja -> I would remove "Polska" from there. "Rzeczpospolita Galicyjska" sounds better IMHO. Though I don't understand why democratic GLM is called "Republika" and presidential dictatorship one is called "Rzeczpospolista" since they mean the same. Current Republic of Poland is called "Rzeczpospolita Polska" in Polish, so I'd change all "Republika" to "Rzeczpospolita".
Poznan:
Why the hell Poznań uses German names!? Doesn't make any sense.
Pomerania:
I'm not sure what Pomeriania is supposed to represent. If Kashubia then well... I unfortunately don't speak Kashubian to help. :/
Pomorze - noun, Pomorski/Pomorska/Pomorskie - adjective.
Księstwo Pomorza -> Księstwo Pomorskie
Wolna Socjalistyczna Republika Pomorza -> Pomorska Republika Socjalistyczna sounds better IMHO (Also I don't recall using "Free" in the name of any of Polish state, so I removed it as it's pretty strange.)
Wolna Republika Pomorza -> Rzeczpospolita Pomorska

I dunno I don't know German, but I agree with the use of German names.
Silesia:
When it comes to Silesia a case could be made for both German and Polish naming... But for fun I'd suggest Silesian names. (Yup, there's Silesian language, which isn't too unified but still...)

Rzeczpospolita Kongresowa -> "Rzeczpospolita Polska" would be OK too but as the game treats free Congress Poland and Poland as two seperate entities it can stay as is to differentiate them.
Poland:
It's OK.
Polish-Lithuanian Commonwealth:
Rzeczpospolita -> "Rzeczpospolita Obojga Narodów" would be a full name.
Rzeczpospolita Ludowa -> "Rzeczpospolita Ludowa Obojga Narodów"
"Rzeczpospolita Polska" -> WTF!? What happened to Lithuanians!?
Państwo Polskie i Litewskie -> "Państwo Polsko-Litewskie" I don't think that it sounds good, but I don't have a better idea, nor I have some examples from history of what could fascist Poland or Poland-Lithuania call itself.
Królestwo Polskie i Litewskie -> WTF!? PLC was a monarchy, yet it was called "Rzeczpospolita Obojga Narodów" anyway...
PS. I didn't check if the game is able to show all of used diacritics.

I appreciate the help you have given me for my mod. Regarding why Poznań had German Names in my mod was because depending on the mod you were using for Vic2, Poznań tends to be a client state under Prussia so giving it a German Name at the time seemed good until I realized that it would look bad if Poznań became independent and kept the German Names (Sadly the names aren't that dynamic in Vic2 so it can't change depending on who owns it).
Yeah, I know. It's kinda a shame since some mods add an decision to establish Poland-Lithuanian-Ruthenian Commonwealth (pl. Rzeczpospolita Trojga Narodów) without tag switch. Some mods even incorrectly use it's flag for normal PLC. Though I'm not surprised since Poland is very unlikely to appear and even waaaay less likely to form PLC.
