The creator of this "mod" is wrong. This is NOT a mod exactly - it is an add-on for Half-Life 2, in the form of a single level. This level is currently available at Gamebanana.com - where the creator there is also wrong. However, at least it's archived somewhere.
Pretty fun little mod! Here's a full walkthrough video I made of it.
