Toothbrush Mod is a Half-Life 2 Roleplaying Mod. It will be extremely realistic and if you play it too much you might forget you are playing a game. There will be two types of memberships. One is free, but there might be noobs in the servers. Another membership option is that you pay $1 a month and play on noob-proof, no lag, good admin servers. The mod should be entering BETA soon.
