Most likely due to newer version of base Doom 3, need to use included DLL or Moddb.com for compatibility.
The add-on is done well and professionally. Only five levels, but they are quite large and many cutscenes. At the last level for a long time could not find a exit - it was necessary to jump onto the shelves and crawl along the ventilation.
