I have always been bugged by the fact that the first weapon was a pitchfork. IMHO, it's completely out of style with the rest of the game. I would have understood a shovel (still meh, but at least it's in style with the protagonist starting the game at a graveyard), but pitchfork?! So I always wanted the "melee weapon" to be an axe or a scythe. It suits Blood more. And now I can have it :)
The sawed-off shotgun from Blood alpha is also nice (looks much better), as well as tweaks to Life Leach and Plasma. A pity that a revolver can be inserted only as a replacement for Tommy, I would have preferred it in place of the Flare gun, but I suppose it's an engine limitation.
Good weapon mod, for the good old game.
