You can't even kill them without summoning the Town guard, and then they will just put you into a even more annoying instance where you do even less stuff.
And rumour has it, that if you don't do the Repentance questline, the Admin will deny access to the secret endgame super level, and will just throw you into a perma-combat zone, with a buffed up version of the Town guard, with no ammo.
Yeah life sucks from a gameplay perspective. Heck it's not even properly balanced and it's completely p2w. Also you can't use an outside currency to pay for stuff so all you can really do is hope that the RNG doesn't screw you over and make you some poor kid in Africa or something.
Don't forget the heaps of annoying NPCs.
