Impossible, nobody has ever gotten hurt in this anime lol Now that I think about it, I never understood why. I mean, they are fighting with tanks and real ammo, and nobody gets hurt. I know this is an anime and not reality, but is there any logical reason for that? Maybe the tanks are super armored, but they actually are WW2 tanks lol, So I don't know. This is not criticism, just something I wondered.
In the anime, they explain that the crew section of the tanks are extremely armored so that any shell impact will not go through it, even at point blank.
actually no, they are not using "real" ammunition.
as Tankery is treated a sport, the weapons used are standardized by the League. they are actually non-lethal and the "damage" done is calculated by the system installed in every tank. like Urdnot669 said, each tank covered with special carbonite plating to ensure the safety of the crew inside. the shells are also non-penetrative so it just hit the armor (some force might applied) and puff the explosion (also explains why every shot explosion are same regarding ammo type). so the whole thing is like a glorified paintball game. thus when the tank were hit, the passenger still safe. although possibility of being crushed by a Maus is something else...
yet, as any extreme sport in real life, accidents still might happen such as minor injury, interior catching fire, or accidental death. in addition, Crew Exposure is actually not advisable due to safety reason, despite some of the character doing this (Miho, I'm looking at you!).

If I remember correctly, this shot of the anime was a miss from an IS 2 hitting the ground beside it and the force rocks the tank.
The inside of the tanks are all lined with some magical graphite armor or some crap I can't remember. It was explained in the anime at some point. lol
What a tank really looks like getting hit. Youtube.com
yea they have some more armor, but they always look outside. im suprised they never got there heads shot.
