DoomGuy is compelled to return to Hell in order to prevent Hell from contaminating reality too much. He battles against waves of the bloodthirsty attackers, but he only manages to stay breathing. He has to overcome riddles, fight demons with bare ammo, and avoid trap hordes at all times. It's like a chain that is impossible to break but extremely agonizing to attempt.
