Are you loading the mod while in the main menu? If not, try that! That bug is something that could occur if you load the mod while already in-game.
I'd recommend asking at the RepZ forum. I'm not up-to-date with the current state of their client.
There's a couple reasons why it's not working: First and foremost, using the modloader on alterIW is pretty much the same as loading it on steam - certain files won't be loaded. Try using the "fs_game mods/moddirectorynamehere;vid_restart" method instead. Secondly, back then, this extended modding support that is required for this mod hadn't been yet introduced, meaning it still won't work properly, as it will lack some features like the armories.

Yes and no. You'll get banned if you load this mod on the steam version of MW2, where this mod won't work due to modding limitations. You can't load the required single-player maps in the steam version, for example. It was meant to be used on "Fourdeltaone", which has been down for a long time now. There might be copycats of the project out there, though.
Short answer: Yes, and the mod won't even work.
Fourdeltaone, originally alterIW, brought better modding support for MW2. The Steam version can only run modified .GSC files through third party tools. This mod is a bit too advanced for that to be enough, as it includes things such as custom menus, textures and sounds.
That's something IW made for the littlebird ride in the gulag mission. It's hard-coded, as far as I know.
This wasn't much of a mod, though. This was mostly a way to re-enable features that were left unused in the final game. Ubisoft wouldn't have needed to do much to include this in the final game... It's suspicious, isn't it? The reason is probably something else, perhaps they were paid by the next-gen console companies to not make the next-gen consoles seem inferior. After all, customers paid a lot of money to have the "best graphics available". It would raise anger if they were to find out that next-gen isn't so next-gen after all.
Being a former modder and a huge fan of Ubisoft's games, I'd love mod support. But I highly doubt that'll ever happen. The closest thing to modding we'll have in the near future will probably only be mods like these, configurations and value changes.
Multiplayer vehicles will probably be a tough challenge, just a heads up! Though, bipo somehow managed to make almost fully functional vehicles, so it's not entirely impossible.

You're really good at this! I've been following this project since it was posted here on MODDB, and damn, you've got some talent.
I've got some constructive criticism though; for example, the music doesn't really fit well when the level changes to night. Also, try to add some more environment sound effects. And of course, as people have clearly stated on your last update, the footsteps destroys the atmosphere :p
But of course, that's just my opinion.
