I remember I had to tune a couple of sounds down because they were overwhelmingly loud, but I can't remember if the machine gun sound was one of them. Sorry. I'll try to take another look when time allows it.

Technically you could combine the maps for both Encounters and write an extended gamedict, but even then, there's a Third Encounter flag that can only be set once per gamedict. You'd end up either playing the Second Encounter with Third Encounter rules, or the other way around.
Go to Addons and download and install the Music Addon. Reportedly it fixes things, I don't know.
Try deleting your config.wl6 file

You only need the base MacenWolf and the relevant addon. The full base version of MacenWolf (Second and Third Encounter) is available at the top of the file list, right below the patches.

For extra frames I think you'd have to edit the source code. Unfortunately I can't help with that.
I can't reproduce the error, but you can start troubleshooting by deleting the config file and trying again. Other than that, maybe try reinstalling your DirectX or GPU drivers.
Tech support tells me the left mouse button should work in-game, the menu just doesn't display it correctly. In my brief testing, both left and middle mouse button worked correctly after being assigned.
Either way, I have uploaded a fix. The mouse menu should display buttons correctly now. Please download the game again, replace your current copy and check if everything works. Let us know in case buttons don't work in-game despite the fix. And thanks for playing.

I'm sorry, but we're not able to replicate this bug. The kill count and enemy count options work fine on our end. I think we would have to see your configuration file to determine what's wrong.
