To cheat I think you actually have to finish the game atleast once on any difficulty.
I'd have to double check that to be sure.
I don't think it will ever happen. I did port it to SDL a long time ago, atleast partially, but it was never fully finished. Maybe one day.
probably easiest way would be to set up a batch (.bat) file and run that.
It's been a while, but to actually able to cheat in this, I think you actually have to finish it once on any difficulty. Atleast it is so in Spear Reloaded.
DosBox can be tricky to set up, but it's true I used DosBox to playtest it because I was on Windows 7, I even used DosBox to run the compiler. Maybe you have too low CPU cycles? I always used CYCLES=MAX.
It does work, there are two major requirements:
1. AUDIOT.WL6 and AUDIOHED.WL6 files from full version of Wolf3D
2. This is a DOS game, so either DOS emulator or native DOS environment is required.
Besides this file is only a patch, you also need the full mod to play this, also available here.

We made a Wolfenstein mod called Klooni. It requires you to own a copy of Final Doom or Doom II to play (for copyright reasons).

You can play mods that don't have any source code changes. Mods that have source code changes might not work. Wolfenstein Reloaded for example has a custom built exe and most features built into it can't be found in source ports. Also graphics aren't in the same position in the file that they are in the standard game and that alone would render most source ports, if not all, unusable. Only way to play is to play the game is running the game executable in Windows, Dos or DosBox.
The exe does not contain standard feature set so it would strike me as odd if it worked with any source port.
Are you sure you have the audio files... (Quoting included readme)

I playtested it with DosBox when we were making it and it worked fine. I suggest you try other mods aswell, because it might be the Wolfenstein engine that is not compatible with your computer.
As long you use the original cheat parameter all original cheats work including the 'MLI'.
Thanks for your feedback. I guess you mixed up 1 and 3 in the difficulty but I got the picture.
What about the added weapons, did you like those, where they usefull or totally useless?

You unlock insanity skill by completing the game. But it will only unlock the first episode only, you'll need to unlock each episode for that skill too. I have to warn you though, it's so hard it makes Doom's Nightmare! skill seem like a picnic. We wanted a to create a challenge for those who might find this too easy on all the other settings.
By the way, how far have you played, we're dying to know what you liked or disliked about it?
Some of you might have noticed I updated the mod profile, well I added a release date. Thought that maybe those who are new to this might want to know when this was initially released.
Also apologies that the M1 Garand "facelift" file was forgotten from the patch it was supposed to be included in. And the Walther sound that we had there temporarily for the Luger, I forgot to replace with the actual Luger sound. Sorry.

I've been following the development of this mod from dawn of times, the first one is my number one favorite of all multiplayer titles available. I believe FH2 will knock FH1 from the pedestal, but it will be there on the second position, atleast until (if) there's another FH title.
Looks very nice, can't wait. Impressive scenes with "ooh I'm outdoors" look and feel to it.
But that's a good thing I suppose ;)

I just thought that this project has died all along. Woulnd't be the first time this has happened to a project (I'm not talking about you).
