With the Hammer Editor in the Source SDK. If you want to make a level you will need to have a basic understanding of how the Hammer Editor works.

In case you were wondering what I have been up to.
Play beta 2. It has everything that 1 has and more, plus bug fixes.
That map is fun. The units seems to have some path finding issues. I would make some of the hallways wider and make some of the ramps less steep. It also seems like some of the later spawners are too out in the open. I died a bunch of times, but I did beat the level. Keep up the great work.
The game uses functions for variables that don't exist without ep2.
It would make sense to have co-op. I just have no idea how to do that.
I am glad people like the mod. Since releasing Beta 2 I have done a little bit of work on the mod but not much. The problem is that I can't just push the mod in the same direction that it was going. I can't make it harder by simply increasing the number of enemies. The last level already causes my computer to chug a little.
So, I need to incorporate new types of monsters that are stronger. I am interested in using Hunters. It took me a very long time to just figure out how to use episode 2 content in the mod after all the updates. Because the hunter only has one skin type I can't have the Hunters be on both teams.
I am discouraged by Valve screwing up the engine to add mac support then not making any effort to fix it. Valve was my favorite game company for a very long time. Currently I have much less faith in them. Making a sequel for Left 4 Dead that could hardly be called an expansion was a terrible move. Never making episode 3 sucks. Most importantly screwing up their engine and breaking tons of mods is the worst.
I may feel motivated to work on the mod in the future. Especially if they fix up the engine. For now I am working on learning the StarCraft II editor. Here is an interesting blog post about screwing over the HL2 mod scene. Planetphillip.com
The third level is almost done. It will be available this weekend. Along with some bug fixes and optimizations.
Also you will need to restart steam after making these changes. Sorry forgot to mention that.
I was having the same problem. Updating the SteamAppId from 420 to 218 then restarting steam seems to allow it to run. That is a fast time.
I hate the pointing at stars part. Someday I will endure it and try to play the mod. I instantly downloaded when seeing this mod compared to Skazka Skazok(Tale of Tales). That movie is so great because the metaphors are so complex and left to personal interpretation. The part of the mod that I played was really beating me over the head with seeing exactly what it wanted me to see. Otherwise the art looks really good in the mod.
This mod takes cues from DOTA for WC3. What map is exactly like this? I haven't played many custom WC3 maps just DOTA and a couple tower defense maps.
Unfortunately, my computer is one of many negatively effected by the recent SDK update. Before the update I was building the detail brushes for the third level. Work will resume as soon as I can use the editor again.
In the mean time I have been loving the Star Craft 2 beta.
The zombies are just there to mix it up.
The third level is being built. In it all enemy spawns are using shields. I dislike posting screen shots of things before they have a lot of polish. I hope to have new levels out in a couple months.
The first release will just be single player. I would like to include multiplayer into a future release.

I started using the Hammer editor in 1999, when it was World Craft. I released several Half-Life Death Match levels under the name King. ASDM02 and Shot Cross most notably. swarm is my first Half-Life 2 public release. I have a BFA in Sculpture from the University of Utah. I made several projects in the Source engine for school.
