Wait...never mind. I figured it out. The cape gives you an extra hit point. You normally start with three, but with the cape, you get four.

What's the effect of the Mirror Cape? I bought that instead of the explosive ammo when I met that shop creature before the first boss. It looks cool on Serena, but it doesn't look like it does anything. Or am I missing something?

Ah, I remember this game...or at least the regular "Bullet Candy" that I first bought from Steam. I think I may give this version a(nother) shot one day soon.
