Make sure you create a new world after you start the mod, and then start a new game using the new world. That's most likely the problem you are having.

The game generates the factions when the world is created, so if you try and use an old world the hidden zombie faction that is needed to spawn zombies won't exist.

Some zombies will have a smaller range that they search for targets. This means for every wave of zombies a portion will stay behind until you kill them. If you don't deal with the zombie stragglers they can potentially turn a group of travelers into a zombie hoard when you aren't paying attention. This lessens the immediate threat of a zombie hoard, but makes them more threatening overall.

If you want to amputate a limb you need a bed that is set as a medical bed, some medicine, and a colonist with the "Doctoring" role. Then after you schedule the operation the colonist will need to lie in the medical bed before the doctor will perform the operation. You can select them and force them to lie down to get operated on.

The frequency of zombie raids depends on your difficulty setting, and there is some randomness to it. It could just be that you have a particularly long waiting period between your last zombie attack and the next. I can't really know what the problem is without having more information.
