We used QGIS to get the map's proportions and scale, it's not recommended for beginners, however, you can get very accurate results.

As for the provinces, we simply use GIMP or Paint.Net to modify the province map.

You are playing with the zombie (apocalypse 1836) submod, disable it.

No thanks, we've already suffered enough with banks and universities, hospitals would have very little effect on gameplay, so we'd rather spend those resources and time elsewhere.

Argentina and Brazil are currently going through massive reworks, with more South American nations on the rework backlog, you can expect the Brazil rework to release very soon.
