What version of CoD4 are you playing? Make sure you've installed CoD4 updates 1.6 and 1.7.
For the mod to work properly on a Mac you'd have to start Call of Duty from a terminal and add the following commands after the name of executable (I don't know the exact name of cod4 executable on mac, it's probably 'iw3sp'): +set com_introplayed 1 +set thereisacow 1337 +set monkeytoy 0 +set fs_game "mods\modernhot" +exec modernhot.cfg
For example, if yo have the windows version of CoD4 and are using Wine/CrossOver, the terminal command would be: wine iw3sp.exe +set com_introplayed 1 +set thereisacow 1337 +set monkeytoy 0 +set fs_game "mods\modernhot" +exec modernhot.cfg
As far as I know it should run on Mac just fine, the only problem might be with sounds encoded in .mp3 which for some reason don't play in Mac version of CoD4, aka. no music in intro level of ModernHOT. Other than that everything should work just alright. FPS-vise ModernHOT runs pretty well on a laptop I've tested it on (i3-4158u 2ghz, intel iris 5100 graphics, 8gb of ram) on everything maxed out at 1080p without AA.

In upcoming update I'll probably add the launch script for OSX too, for now you need to start it manualy.
Install the hotfix which solves this and a few other issues. Alternatively you can change difficulty by typing: \g_gameSkill 0 in console _before_ starting a map.
Bugs like that happen in mods of this scale and we're just two people who have their jobs and studies. We've still managed to fix all the most frequent issues and more in such a short time. Glad you're going to replay the mod and we hope you'll enjoy it a lot now!
No console game (which I know) is moddable, this is for the PC.

Shotguns use "spread" fire mode which instantly fires a few hit-scan lasers with infinite speed, when using a projectile weapon it spawns just one projectile that can become a bullet. It is possible to do "projectile shotgun" by setting its fire time very little or to spawn additional projectiles via code.
And I'm really wondering how a new menu design and more ranks make the gameplay better? ^^
It's a bit wrong to assume that these make mod better.

Why would you start Death Run from a server's reserved fs_game directory? Mod will derank anybody who does that as othwerwise you would be able to hack your rank or "boost" level with a friend.
I can't help if the game crashes without mod.
This game looks amazing, and I don't only mean graphics, the gameplay, the features and levels are just great. Although it won't run on my PC but it won't stop me from buying this just to support devs :)
It could be pretty much 'infinite', world could be saved in chunks like in minecraft, each chunk is 256x256x256 (three small ints are it's constant dimensions), two ints (32 of 64) will store X & Y location of each chunk. Each object will be placed inside of a chunk, it's location could be stored as a chunk id + 3 floats. Seems pretty easy, isn't it?
I could go on and write a longer comment about how to do it but it would take me a quite much more time.
