It's in a room with mirrors in a box. You have to solve the morgue puzzle first in order to open up the 2nd (?) floor, then a few more puzzles after that, if I recall correctly.

Happened to me as well. All I did reopen Alchemilla and when I tried to go out the door again it worked. Just keep trying til it works I guess?

I put the second mannequin arm on and opened the little box, but I have no idea what the note inside means. There's nowhere else I can think of going. There's a key behind a fan, but I have no idea how to turn the fan off.

Yeah, I figured it out by doing the same thing a bit later! I'm just surprised there was not hint whatsoever on how to adjust the pipes. I just tried every combination til it stuck. Now I'm lost in the fleshy apartment building...
