Two of the organ implants let's space marines breathe and survive in space.
But a helmet would make your survival rate significantly higher.

Not exactly breathe, but it keeps them alive for a long period of time without oxygen. The mucranoid and multi-lung to be specific.
There's also bound to be an air intake valve linked to the black carapace to supply oxygen despite the lack of a helmet, but that's speculation.
I thought it was just the void shield.
they cant breathe in the complete vacuum of space they will freeze and die eventually but the star ships have a thin vial of atmosphere around them.

Battle of Calth to be precise. Ultras vs. Word Bearers, Ultras get their **** stomped, although Guilliman survives and nearly kills Kor Phaeron.
