Check out this blog, it makes sense as it solves alot of the plot issues at the end of the game, but what a risk for Bioware. I only believe this may be true because the endings sucked and I hope against all odds that Bioware didn't just screw up a great series for nothing.
Oh yeah, and another point, on earth before the beam strikes shepard he has his armor on (whatever you had equipped), but in the final cut scene with the reaper/child/god/VI Shepard only has on his casual clothes. Then in the rubble scene he once again has his armor on.
The rubble scene I am referring to is after the destroy option is taken and you had enough war assets. I presume (as most everyone esle does) that the unique N7 chestplate and dog tags are Shepards.

Yeah, I know, but you have to be either a completionist or very fond of the multiplayer to get said ending.

Since that rubble at shephard survives is mainly concrete i think he never got on citadel in that attack. Could be just overlooked detail by BioWare
Or just want a survivalist ending. Hell I didn't know what the **** I was doing to survive in ME and ME2. I just survived.
