I want to suggest maybe tweaking the drops in the monster mod when running with this mod as for an example, I came across the Imp replacements just dropping a pocket nuke on the very first map instead of maybe a set drop of sorts.

I do want to actually suggest having the option to toggle just using items on pickup (aside from those that have a reserve like the portable rejuv. kits if that is fine) or having it as the inventory system it is now since I usually liked it before it was given the inventory system. Although if anyways, could you have it so that the inventory items give a brief description on their effects when toggling through them? Kind of to refresh me on their usage when coming back.
But speaking of options, I think you may need to organize them into sub-menus as the current options layout being all in one is a bit cluttered.
