The Battle for Middle-earth: Tactics – is a first mod for The Battle for Middle-earth II: The Rise of the Witch-king which main goals not only to add units, heroes and buildings, but also cardinally to change a gameplay and to improve graphics.
Its kinda funny actually how everyone tries to dress Khamul in weirdly shaped crowns and masks while in the same time completely ignoring actual Easterling theme from the movies. :P
It is likely that this design seen in Five Armies is Khamul (unless I'm mistaken and the production have said that it was). The reason the fans and modding teams believe this is Khamul is because it is similar to the Easterling design. =D

Fans also believe that Peter Jackson cameoed as this Nazgul, given the body shape and that he has cosplayed as a character with a similar design silhouette.
