Cultures roman and punic have the ability to overcome cultural requirements in some empires (Gaul, Dacia, Britannia, Iberia, Germania and Arabia for roman, Iberia and Libya for punic), provided that they are independent from mother faction (Rome and Carthage respectively) and they are not rulers of those factions. Cultures that are not listed in any of de iure empires will have ability to create (via decision) their own de iure empires (such as armenian - Empire of Armenia) if they meet the requirements for the decision.

I think you should remove the ability to hold de jure titles for all tribal rulers. The distribution of power was constantly changing in those lands and there was no regional or ethnic identity similar to the Mediterranean peoples, so titular titles would fit better.
The Germanic tribes, for example, believed in the concept of "Heil" (well-being, salvation) and "Ehre" (honour). Honourful deeds were supposed to grant "Heil" to a person or family. Since "Heil" came from the gods, the more you had, the more respected you were. And others were willing to follow the most prestigious ruler.
You could soften that up a bit in the later bookmarks because by then larger tribal confederations where forming, and people like the Burgundians created actual kingdoms within the disintegrating Roman empire.

De iure titles will be restricted for so-called barbarians. I'm planning to connect it with technological development - the same way Paradox associated Military Organization with pagan lands' attrition. Tribal rulers would be barred from creating de iure titles until for example 6 level of this technology, kingdoms from 8, empires from 10.
