After doing some additional experimentation with demos in vanilla Doom 3, it appears they had their share of issues even when the game came out. Fixing the additional issue of crashes for PD3 when playing demos may not be worth it (assuming it's even possible).

Additionally, I ran into a bug that I thought was just confined to the "more monsters" addon; in Administration, there's a part where you cross over a room where Swann and Campbell are working on a computer. After some dialogue, the two run out of the room through a door. However, when I played through this level, Campbell ran to the door and then ran around in circles.
EDIT: I did not have the "More Monsters" addon installed when this happened on my current playthrough.

Also, I reinstalled PD3 and tried going through the weapon melees again. This time, all of them worked without any game shutdowns.

I'll attempt to reinstall Perfected Doom 3 and see if that fixes anything in regards to the melee attacks. As for the reflections, the bathroom in Mars City and Mars City (Revisited) doesn't show the marine's reflection, unless you are in thirdperson. There's also a mirror in a bathroom later on in the game (I think it's in the Alpha labs towards the start of one of the levels) that has the same effect.

By demos, I'm referring to the ability to record gameplay using the game's engine. In the console (~ key for PD3), if you type "recorddemo <name>", the game starts recording a "demo". When you type "stoprecording" in the console, the demo is finished. Then, if you type "playdemo <name>", the game plays the footage of what was captured during the demo. I can record and play them back fine in vanilla Doom 3, but playing them in PD3 results in the game crashing.

Hello VGames; I enjoy playing your mod. The graphical improvements and gun changes were exceptional! I also loved how you extended the Hell section of the game with the additional three levels (including the Icon of Sin). I'm looking forward to seeing the Spider Mastermind when it's fully completed.

There's only a small handful of errors I come across. Aside from the occasional crashing, sometimes when I use the melee weapon attack for the weapons, the game session ends with the console showing something about the meleeDef not being found for weapon_<weapon here>. This tends to happen for the shotgun, machine gun, plasma rifle, chaingun, and BFG. Reflections in the bathroom mirrors don't work unless you're in thirdperson, and demos don't seem to work (I can record them fine in-game, but when I try to play them the game crashes).

I'm enjoying the mod so far; the only complaint I have is that there are issues with scripted NPC movements; for example, in the section where Swan and Campbell are running out of a large computer room in Administration, they don't leave the room normally, and instead end up running in circles trying to leave. This also happens with some enemies, where they just run around in circles.
