I do understand your concerns, but blood impacts I have right now are made to look hollywood-esque and also to be clearly visible almost at all times. Darkening them or reducing their size will make it hard to see hits at distance or in bright daylight.
I updated the mod with a more consistent file structure. Installing it should now be less confusing for newcomers to modding.
Place everything straight into your gamedata folder.

Nope, it doesn't. It's a simple content replacer, nothing more.
Open ayykyu_fp_death.script in your gamedata/scripts folder. Remove get_console():execute("load_last_save") line in function final_death() to disable the auto-load.
Replace campfire stories and misc voice clips with empty files and you'll have a perfect mod.

Just have a look through what's already there. The code has explanations written all over it and the sounds are easily accessible.
This is a framework first and foremost, English voice lines are pretty much placeholder and were used because I myself prefer my player characters to speak English. You are very welcome to make your own voice packs, as long as you preserve the original base framework file structure, keep all the code comments where they were and give proper credit where it's due. You can always contact me on Discord if you need help creating a voice set for this mod or if you simply want to tweak something.
Realism isn't what I was aiming for, style and feel is.
In its current state this mod urges you to get rid of rads as fast as possible to stop the health drain and also to give a faint feeling of sickness and disrepair with its "rustic" colour scheme.
