^ The sample rate does need to be 22k but it does not have to be mono. Many of those limitations you mention are part of QVM and not roq2 in general.
Do not waste your time with Quake Video Maker, the best route for me after many different paths attempted is by far: uncompressed avi > ffmpeg (using roq as the output format, at 30 FPS)
I've gotten some awesomely smooth videos in game, it's a really badass feature for such an old engine. Also 7th Guest was awesome :)
