To make a 2D Image out of a 3d model you need to render. And a render is an 2D image .. lol
I don't think so. Look at the pipes on the wall, you can see individual faces on the model. If you;re talking about the mouth then I have no idea.
I'm pretty sure it's 3D - look at the specular highlights on the teeth. They would look really out of place if it was just a 2D image and didn't change with lighting.

Lighting says to me, 3d, but I wasn't sure at first...it looks amazing either way.

This is one of the first in game screen shots ever released for NS2, I don't know why you think you've seen it in the Skulk reveal but I'm sorry you're misinformed.
As for being the real deal, I'm not sure what you classify as real but that is an unedited in game screen shot of the pre-alpha build.
Now I understand why you think you've seen it before, what you saw was concept art: Unknownworlds.com
The vent you see in game is based on that concept art, just like most of their assets.

The perspective is from inside the mouth because you can see yourself biting Marines and generally it looks more interesting.
They're not actually meant to have eyes in their mouths, although they could considering they're fictional aliens.

it would be pretty amusing if the alien decided to close his mouth through out the entire game, anyway, im pretty sure the actual mouth was added after the render, too cool to look like that in game
Thats pretty sweet. Though I have to ask one question. Why are you seeing things from inside the mouth?
It does not have eyes inside its mouth, the player prospective was placed inside the mouth because its just lame not being able to see your self bits things, it has a much much better feel having the camera placed inside its mouth than not. It's tried and tested.
Also yes it is indeed a 3D model. The tongue is animated too.
