Hey ! Yes it's 3D realtime, about technic it's just a basic flat shader receiving shadows. The fake shading work is done previously during the drawing process on the textures.

Some people will spend hours generating normal-parallax-occlusion-subsurface -ambient-accumulation textures, others will make hand-drawn textures... this one goes for the hand-drawn, hands down (pun intended).
Is this realtime 3D? If so, then it's one of the best toon shading I've seen...or just good art. Nicely done.
