Do the difficulty commands apply to only Heroic and Legendary difficulties?
Pretty much. You can play on easy as well if you'd like. easy = easy, heroic = hard, and legendary = impossible. Just type that command in the directions when you are in the UI and then load up one of the maps.

I have found a bug that the ammo counter and the shield and health bar seem to be part of the distant terrain, not sure what is going on there.

You need a Halo PC key. So yes, you should have the original Halo to install Halo: Custom Edition.
I just did an entire Let's Play for this campaign for my channel. It was phenomenal. The Beauty in this was amazing. Such hard work.
For those of you who want it, here is how run this at the actual resolution of your monitor.
The "Hopefully" Idiot proof guide.
1. Create a short cut to the Haloce.exe.
2. Right click shortcut, and open properties.
3. select the line labeled Target.
4. Using the arrow keys, go to the very end of the text in the target field.
5. Making sure that the text is *NOT* highlighted press, spacebar, and then paste the code for your resolution.
If you have a 1080p (1920x1080) screen use this:
-sw -vidmode 1920,1080,60
If you have a 720p (1600x900) screen use this:
-sw -vidmode 1600,900,60
If you have a resolution of 1366x768 use this:
-sw -vidmoe 1366,768,60
If your resolution is not listed, and you can't figure this out on your own, then respond here, and I might help you.
Not sure why I said 720p, 1600x900 would be 900p but it's never referred to as that.
