You're Mike Sullivan, a Black Ops captain that leads an operation on an island with weird, unknown activities. During the operation, your team suddenly gets deadly diseases, and they start to disappear aswell. The deeper they got, the more deadly it got. In the end, everyone either died, or disappeared... except YOU. When you start to wake up, you realise you're not in the middle of the jungle anymore... It's far worse, an abandoned Laboratory deep underground, where illegal experiments are made. But... there's a weird unknown specimen that managed to escape the room where it was trapped in. That specimen will follow you, and possibly try to rip you apart. Enjoy trying to escape the labs..

The left eye would be hard to do in Source with skinning.
I've seen far more difficult and complex things modeled and textured. This would probably be a piece of cake to anyone who knows what they're doing.
They could make it a physics object. Characters in DoTA2 have plenty of them.
It also will have ;-).
