it changes the first level around by adding the thing creatures and moving/taking away some items also all credit goes to MegaInfernal1 sometimes know as just Infernal and yes the game goes as normal after you finish the first level

just extract the zipped file then replace all the lvl files from the main game folder with the lvl files from this mod to install it. What it changes is the first level around by adding the thing creatures and moving/taking away some items also all credit goes to MegaInfernal1 sometimes know as just Infernal
