Account system: Yes!!! :D
Just nicknames: No, I'd add a short description too and maybe the whole overview of the map OR a selection made by the user - you could chose what you wish before you upload it.
Anonymous: I think it's right that the creator of the map is shown on the download page. As for online levels, that sounds like a good idea :) I wouldn't really delete maps with poor ratings as that's just mine or your taste of the map. I would however prompt the creator that the rating is low and if he wishes to update/delete it.

Your maps would have the option to update/delete the map.

You can't if the mod isn't on the Steam servers. Only some HL2 mods are on there, no Portal mods(yet) :(
