There isn't actually a bug, but the way it works is misleading. They fill a set amount of health rather than the full size of your bar, and it will use more than one of them to fill as much as it can. You can restore these at save points if I recall, so it sort of balances out.

While I can't make the game adapt to other layouts due to the nature of the library it uses, there is full keyboard reconfiguration available through the INI file included with the game.

The original game's vsync feature is actually incredibly broken on any refresh rate except 60Hz. I've tried to fix vsync properly in the game's engine (very similar to the ones used by the Gundemonium Collection), but I hadn't had much luck. That's why the timer/vsync option was removed from those games and this one. However, it's something I'm still looking into. Until I can find a solution, try forcing vsync through your video drivers. That might well work, or at least make it a bit better.
