I had a feeling part of this was intentional, especially on the smaller maps, and especially on Siege of Khe Sanh, but i think the AI on these maps can still be changed or improved.
Take a look at the other conquest assault maps where the defenders operate completely differently than from the maps i listed before. Operation Game Warden, Across The DMC-1967, Battle of Hue, Quang Tri-1972. These maps where the defenders are already holding the majority of points, when the game starts they already start moving around from point to point. They may not be on the offensive initially, but they will attack and recapture points that have been taken from them. Whereas on the maps i listed earlier, it’s as if they don’t even try.
I think even the AI in Siege of Khe Sanh can be altered in some way. In Operation Cedar Falls, which i think is in a similar gameplay situation where they have to defend a small area from multiple sides, the AI actually moves around from point to point, even in such a small area. on Siege of Khe Sanh they just don’t. You’ll see US soldiers standing around doing nothing while the NVA surrounds and closes in on them.
The defenders move around when they’re combatting enemies that approach them, sure, but never to move to another point (that they already owned), or even recapture one taken from them. So when the attackers capture and hold the majority of the points, the defending side literally can’t resist or recover, since they don’t move around or try to recapture points.

This issue i'm having shows up exactly the same way in both Outpost Red Haze and Cambodian Incursion. Most of the AI on the US side when they spawn just don't move at all (and tend to be clumped together in big groups), unless they are approached by enemies, and even then usually still don't deviate much from their current positions.
They don't attack or go on the offensive, they don't try to recapture any points taken from them. This results in some fairly one-sided battles and can pretty much create a guaranteed loss for the defending side.
I haven't really payed attention to what other maps are affected by this, but if i had to guess it's probably an issue with the 'conquest assault' type of maps in general, where one side starts with the majority of cap points in which they have to defend. Same goes for Fall of Lang Vei, they don't just idle on the western-most point, they idle on all points.

EDIT: i've made a list of all the maps where i seem to have this issue.
Battle of Ap Sac (issue seems to fix itself once attackers cap neutral points)

Thanks for the response—and, understandable. at least now i have enough clarification as to what i can expect from playing.

on another note, is there any possibility that you’ll be able to let players increase the max player/bot count above 64 when creating matches?

I was thinking more like patch notes or something. Like in the ‘special features’ text file, so we could see what individual changes were made, and what has been altered from the base game

Well, i was playing with v1.02 installed—so i played multiple more games to make sure, reinstalled v1.02 just to make sure (when viewed in the custom games menu, Operation Remembrance shows to be at v1.02, so it must be installed correctly?), and i’m still having the same issue, soo

On Fall of Lang Vei, at the start of the game, most (if not all) of the AI on the US side that spawn in on the western-most spawn point all crowd into one spot, and remain idle doing nothing until they are approached by enemies (bots doing nothing seems to be an issue in this particular spawn area in general, even when enemies approach).
Similarly, once the NVA side captures the middle point, a few NVA soldiers will also stand around idly doing nothing on the point.
