Aardappel gave it his blessing today so this weekend i'll finish testing it with the Frikbot and release it. The only major change from normal SDQ is the removal of gib eating and solid monster corpses. I am also working on making demons, shambler, and vores spawn in harder skill levels. I'm using qip_quake from www.inside3d.com/qip and setting "sv_entities 1536" at the command line. Telejano seems to work well, but there's a problem with the Dark Places engine and per-kr's devastator model. I'm not a modeler (i'm barely a coder) so I have no idea how to fix it. I would not attempt to run this with old vanilla quake. Anyway, have fun!
