Author's said that he wanted to create a world predating the movies. So this would be orthanc, only a few hunderd/thousand years before the movies took place. Naturally they'd look different.
Indeed, as you say, this is intended to represent an Isengard before the War of the Ring, established shortly after the landing of Númenórean refugees in Middle-earth. Hence, unlike the Orthanc of the Third Age, this has outer walls beyond the main ring housing those citizens unable to live in the underground caverns, walls which were later destroyed in the Dunlending invasions of the early Third Age.
