This new version (3.1, "Reopened") is specially rebuilt for ATDD version 1.5, and may not be as you remember.

Amnesia potion recipe:
- 1 part rose water
- 1 part vodka
- 3 parts pink lemonade
- 1 teaspoon honey

So there are only 2 endings? I used the poison bait and it didn't do anything in the end. The only endings are either destroy the orb or drinking the amnesia potion. Am I missing anything?

Those are the 2 endings. The orb isn't destroyed though... I'll explain it all in the walkthrough/commentary that I'll be uploading in a day or 2.
I can't figure out the library puzzle. I have 11 notes (not including the one found in the library). I'm guessing you find the final note in the library behind a book case or something but none of the dates on the Annals note work when inputted with the sliding books. The only thing up the spiral-staircase is a closed door.
I'm guessing all that is left to me is the trap door but I'm getting the idea that that is a bad idea.
The trapdoor is always a bad idea! Is Burke ready to face is fears? Probably not.

(The book puzzle will only work once you have the answer. It was a touch decision to make, but I wanted to stop people from guessing the solution before they've got the clue. In other words, one of those dates will work, but only once you know which is the right one.)
