I'm not totally familiar with the Witcher series but here I go.
1. If we're in some kind of medieval setting, probably a big Scythe because I'm pretty predictable like that. They are a pretty interesting weapon, being difficult to master but very flexible when mastered.
2. I'd probably just kill whatever without much regard for reimbursement for my troubles. Though I'd take a much more peaceful approach to things to try and gauge if there is another solution.
3. Just call me Templarfreak! ;)
4. After thinking about it for a while, I can't come up with anything really. There are some freaky monsters from video games out there, but none really keep me up at night anymore. Something that is completely hostile, highly intelligent, and likes to **** around would probably be a close bet though.

1: a crossbow with some sort of mechanism that lights the tip of the bolt on fire

Question 01: If you were a Witcher, what kind of weapon will you use against monsters?
Answer 01: If the creature is not sensitive to silver i'd use a simple steel claymore, i love how it looks, how it feels and is good enough when you fight bigger and stronger monsters, while otherwise short or longswords would not be enough.
If the creature is sensitive to silver, i'd use twin swords made from meteoric iron, and then coated in silver and inscribed with magical runes.
For a backup i'd like to have some kind of a mini crossbow attached to my forearm so i can take out smaller and faster creatures more easily.
Question 02: Witchers get paid for killing monsters. Would you help people who need help, who can't pay? Or simply move on to the next customer?
Answer 02: The world of "Witcher" can be a truly brutal place, but still i have to live off of something, and killing monsters is as good as it gets. After all i was made to do it, so why not. Of course i would help someone who can't pay, even a loaf of bread can be a good payment. In time my reputation will grow and sooner or later a rich customer will ask for my skills because there aren't that many "Witchers". And the best thing about having a good reputation is that the ladies will throw themselves at me. There are always damsels in distress :P
Question 03: What name do you want people to call you, since you are a Witcher?
Answer 03: You know what, it doesn't really matter. "Witcher" is as good as any. It short, it sounds scary and it's easily said, it simply flows from your tongue. But hey if the people come up with some awesome nickname, i wouldn't mind it just means i'm doing a good thing and my reputation is going up.

Answer 04: Honestly i was always "scared" of ghosts. I mean think about it, they are incorporeal, you can't hurt them yet they can still rip you apart. They can move through walls and are extremely fast, to the point where it looks like they just teleported behind you in a blink of an eye. Worst of all they can make some truly bone chilling faces. This is a bad example, i couldn't find the scarier one on imgur... I.imgur.com
1. I would use cold steel hand and a half sword, because its relatively light weight and mid sized blade make it possible to wield one handed so that it could be used along side a shield, but it is also possible to deliver devastating two handed strikes.
2. I would of course help someone who needs help but cannot afford to pay, however i would not make a habit of it because if you're too lenient people will take advantage of you, and the same note you cannot be too merciless or people will fear you rather than respect you.
3. I would use the title "White Knight". which is defined either as "a hero who comes to the rescue" or "a beleaguered champion who fights heroically for a cause" because it is somewhat modest and fairly easy to remember, and it is not tied to a single man so that i could train a successor to take my place.
4. By far the single most terrifying thing for me is people. But let me explain people with suffering from psychopathy who can apply logical thinking, but lack all sense of empathy and could kill you and never display even a hint of remorse.
No link because indisputably the most dismaying part is that anyone could be a psychopath, the cashier at the store, the man who sat next to you on the train, even people in your own family.
*This is not to imply that all people who suffer from psychopathy are deranged killers, just that to me the thought that there are people who can kill without a second thought is frighting.

Question 01: I would use a silver edged hatchet or small swords in dual wield fashion.
Question 02: I would kill monsters regardless, there's loot for money along the way.

Question 04: A Necromancer of unholy rapport.
1. A Spear, spears are badass, you would be stupid to not have one to kill from a reach away.
2. A contract is the only thing I care about, contracts and pay, I live on from my clients' tasks
3. Volk, a pretty simple name which means Wolf in russian, would fit for how I would go on about encounters.
4. The Gazebo, the worst thing in all existence possible.

That's only reply to the Rule 3 part, this contest are only under the Rule 1.
Question 1 = A Bastard Sword for most combat, Short Silver sword for close quarters combat, Brass knuckles hidden under gloves at all times. Dagger on lower leg, and Bow and arrow (Silver tipped) if needed for long range.
Question 2 = I would help who ever needed it. Even if there was nothing in return. Always.
Question 3 = The Witcher of Death.
Question 4 = The monster i fear, is not one i can discribe, for it is me, and it isn't. i am it, but i'm not, It is what i can become, but refuse to be. I am He, and He is I.

1.A silver sword with some kind of decotion. Maybe one from the Elven ruins in Toussaint.
2.Killing monsters is a job. I don't do jobs that don't pay.

4.A monster? I think a cunning human could be a bigger threat than any kind of monster. Monsters are instinct creatures, humans are not.
1: My main weapons would probably be the same swords I described in the Darksiders competition. Their description again:
2: Of course yes. I also wouldn't charge too much of normal people. Enough for a decent survival is good enough. (**** the rich, though. They will always pay. A lot.)
Plus, even if I didn't help just because I am a good person, seeding good relations and alliances with people here and there would be good for me. Could make my life and assignments easier and, who knows, maybe one day I'll be the one needing a friendly hand.
3: The... wait for it... BioDestroyer. (It already sounds awesome and I already use it. There is no need to come up with something else, especially since I'm bad coming up with names.)
4: I can't come up with anything. Maybe I don't have one or maybe it's one of those cases where you don't know until you see it.

2. Sometimes it is acceptably. If you are some kind of bounty hunter food and shelter could be convenient.
3. Something simple, like "Predator".
4. If you can imagine it, it's already not that scary. And this is why it should be shapeless/shapeshifter.
