If you don't understand the game that means you are doing it wrong. I didn't understood it for the first few tries, but after I started to learn it for a few minutes (20-40) I was able to start to play with it with the help of the wiki.

But be warned if you are just a dirty casual this is definitely not for you, because this game is THE definition of gameplay itself. There is plenty of tutorials, tilesets and other stuff that helps you to understand Dwarf Fortress.

And why would you play it? Because of building and doing whatever you like, because of epic and awesome stories, because of brutal fights and weaponizing almost anything(like cats). Sure it doesn't looks good at first, but it will teach you what gaming is about. Gaming is not about graphics or casual wining, it's about having FUN, and this game has more of it than any other.
