This mod from Asheer makes changes to monsters and weapon stats to increase difficulty.
Harder Mobs V.1b (Credit for flashlights on Machine gun and Shot gun go to Glen Murphy)

The purpose of this mod is to make the monsters of the game slightly harder and the guns slightly weaker so the game
itself becomes more of a challenge. If your like me then you think the game is a little to easy even on nightmare
and the mode before it. well this mod will change that so you stand a higher chance of being ripped apart by
all the minions of Hell.
Extract and copy/move the Harder mobs (beta) folder into the Doom 3 folder (C:Program FilesDoom 3 is what it
should be by default). Launch Doom 3, click mods, load Harder mobs (beta) and enjoy the slaughter. note that any
saved games you have will default to the begining of the stage your on. If you remove Harder mobs (beta), then
they will start from where ever you originally were in that stage.
Monsters - For monster changes please refer to the Monster Specs.txt
Pistol - Clip size raised to 14 bullets.
Damage changed to 10 for more of a challenge.
Fire rate changed to .3 (quicker fire rate)
Shotgun - Clip size increased to 12 shells.
Reload rate increased to 3 slugs per shell. (realism not intended.)
Damaged changed to 12 for more of a challenge.
Machine gun - Clip size increased to 80 bullets.
Damage changed to 8 for a somewhat realistic feel to the weapon.
Chain gun - Clip size increased to 80 bullets.
Damage changed to 15.
Grenades - Changed the amount that can be held to 100, lowered to the fuse time and raised the damage to 150 to make them more realistic.
Plasma gun - Clip size increased to 60 plasma shots.
Damage changed to 14.
Rocket launcher - Damage increased to 190 to help with critical moments such as being surrounded. (dont blow yourself up in the progress of trying to stay alive :P)
BFG 9000 - No changes have been made to the gun since the gun is plenty strong already.
Soul cube - Damage drasticly increased to help kill the Cyberdemon but this was done at the cost of souls needed to
launch the attack. (Damage increased to 2000 and number of souls to needed increased to 10.)
Chain saw - Damage was increased to 60 to help with the critical moment of low ammo.
Flashlight - No changes made
Fists - No changes made
Player - Max health increased to 250.
Max armor increased to 300.
Stamina made to be unlimited.
Run speed and jump hight slightly increased.
Max ammo for all guns increased. (Running for your life and not running out of ammo are a must for your survival.)
Items - All med packs, Security armor and armor shards have been increased to help you live a little longer.
Ammo - Large and small ammo packs have been slightly increased.
