"last stand" significa literalmente "ultima oposición", dado que se supone que es un apocalipsis zombie yo lo interpreto como una "ultima pelea por sobrevivir".

the usermaps cant work from the start new server menu, you need lauch a game, when you are in game, open the console, type "/changemap mp_surv_mapname" (mapname is a usermap name), and now you can play. I hope this helps.
you may also type in the console /map mp_surv_lastdefense
