A survey on Level Design in games. How players, modders, and professionals experience it.

I am currently doing a survey on level design in general. How players, modders, and professionals experience it. Most focus is on FPS level design (aka the historical big three: UT/HL/Quake).
Help me portray an accurate picture of the state of level design! The results will be read by a good number of professionals and modders across the communities.

Hm... didn't expect this kind of a survey. I thought more it would be about how mapping is done ( like what kinds of maps are good and which are bad for SP and MP ) instead of this extreme questions but maybe this yields something useful.
The only thing I didn't like was the last question. It sort of makes it so if you are a level designer/artist, you aren't a gamer, and vice versa. I think it should have been a multiple check question, and have gamer as a separate entity.
